They came out of a real problem in our own practice. Most CMS questionnaires are built for immigration, but they tend to cover more ground than any one case needs — and they have no mechanism to catch incomplete histories. A client can submit with a gap in their address or employment record and the form accepts it without a word. That becomes the attorney's problem to chase down.
The back-and-forth over incomplete address and employment histories was eating hours every week. We built these questionnaires to ask only what each case path actually requires, with real-time gap detection that flags missing history before the client can submit.
Full practice management platforms are comprehensive but expensive ($55–$200/user/month) and their questionnaire tools are general-purpose. They're not built around the specific questions that I-130, AOS, or DS-260 cases require, and they typically store client data on the vendor's servers — which creates GDPR exposure if you have European-based clients.
These questionnaires are the opposite: a focused, one-time-purchase tool. You own the file. Client data never leaves the client's browser. There's no platform to log into, no contract to cancel, and no per-seat fee as your firm grows.
You send the client a link to the questionnaire on your website. They open it in any browser, read the GDPR privacy notice, check the consent box, and begin. They work through the sections at their own pace — the form auto-saves every few seconds, so they can close the tab and come back later without losing their work.
When they reach the final section and everything is complete, they download a structured summary as a text file and email it to your office. That's the full workflow — no login required, no account to create, no app to download.
These forms were developed using AI, but AI plays no role in the questionnaire experience itself and never receives client information. The finished questionnaire is a standalone web form — once it's on your website, it runs entirely in the client's browser with no connection to any AI system, server, or cloud service.
Client data is stored only in the client's own browser as they type. Nothing is sent anywhere — not to your server, not to ours, not to any AI platform — while the client is filling out the form. When they're done, they download their completed summary as a text file and email it directly to your office. That email is the first moment the information leaves their device, and it goes straight to you.
This architecture makes you the sole data controller for your clients' questionnaire information, which is the cleanest GDPR posture possible. A full GDPR Art. 13/14 privacy notice is built into the welcome screen of every questionnaire and must be accepted before the client begins.

Customization includes your firm name, attorney name, logo, brand colors, contact email, and your preferred language or language pair — the standard version is English/Spanish, but other languages are available on request.
You'll also receive a plain-English software license agreement and a one-page guide explaining how to upload the file to your website.
You upload the HTML file to a folder on your web host and it becomes a page at whatever URL you choose — for example, yourfirm.com/client-intake/. The process takes about five minutes on most hosts (GoDaddy, Bluehost, SiteGround, WordPress, Squarespace, etc.) and doesn't require a developer. We include a step-by-step guide with your delivery, and we're available by email if you run into any trouble during setup.
You have a couple of good options. The simplest is Netlify Drop — a free service where you drag and drop the HTML file into a browser window and get a live URL in under a minute, no website or technical setup required. You'd send that URL to clients exactly as you would any other link. It's a legitimate, professional solution that works just as well as hosting it on your own domain.
If you do have a website but aren't sure how to add a page, most web hosts have a file manager built into their control panel that makes it a straightforward upload. We walk you through it in the setup guide included with your order, and if you get stuck we're happy to help over email.
